Abstract

Postpartum depression affects a substantial share of new mothers worldwide, and the strongest predictors identified in the epidemiological literature -- poor social support, exposure to intimate partner violence, and unplanned pregnancy -- are psychosocial rather than purely biological, motivating a class of interventions built around psychosocial empowerment: peer support, community health worker-delivered structured therapy, group-based prenatal and postnatal care, and targeted self-management programs designed to increase a mother's perceived control, self-efficacy, and social connectedness. This article synthesizes that literature, grounding it in Zimmerman's psychological empowerment framework and Bandura's self-efficacy theory, and tracing the evidence from the Thinking Healthy Programme's origins as a single rural Pakistani trial through its cross-cultural adaptation into routine antenatal care in Turkey and China, alongside independently convergent meta-analyses of peer support, a growing group prenatal care trial literature, and emerging evidence for compounded-risk populations facing intimate partner violence. Read together, the evidence supports psychosocial empowerment programs as a genuinely effective, moderately-sized, and increasingly portable intervention class for postpartum mental health, while leaving direct mechanistic testing of the empowerment pathway itself, dose-response and component-isolation questions, and the field's limited engagement with the structural conditions underlying psychosocial risk as an open agenda for future work.
