Menstrual Cycle Phase and Injury Risk in Team Sport: Evidence Quality Behind a Popular Claim

The claim that anterior cruciate ligament and other injury risk varies by menstrual cycle phase,
particularly around ovulation, has become widely repeated in sports medicine practice and media
coverage of women's sport. The underlying evidence base, however, is markedly heterogeneous: a large
majority of included studies estimate cycle phase by calendar counting alone rather than confirmatory
serum hormone assay, sample sizes are frequently small, and systematic reviews pooling this literature
report inconsistent direction and magnitude of effect across studies. This review synthesises the
case-control, cohort, and systematic review literature on menstrual cycle phase and injury risk in team
sport, examining specifically the methodological quality underlying a claim that has outpaced the
certainty of its evidence base.
