Abstract

Device-associated bacterial colonization remains a leading driver of implant failure, from periprosthetic joint infection after arthroplasty to catheter-associated urinary tract infection, and silver nanoparticle (AgNP) coatings have emerged as one of the most actively engineered surface strategies for preventing that initial colonization. This short communication synthesizes the current evidence on how AgNP coatings kill bacteria, through combined membrane disruption, oxidative stress, and sustained Ag+ ion release, and surveys the coating architectures, electrochemically deposited, plasma-polymerized, pore-structured, hydroxyapatite-composite, and peptide-conjugated, that have been engineered onto titanium, stainless steel, polyethylene, and polymer implant substrates. Where directly quantified, reported antibacterial performance is substantial, including an 83.47% antibacterial rate and a 22.5 mm zone of inhibition for an AgNP-in-epoxy lining on polyetherketoneketone tested against Porphyromonas gingivalis. We compare this preclinical picture with the more mature clinical trial record for silver-coated urinary catheters, where independent randomized trials have established a reproducible reduction in catheter-associated infection, and argue that closing the translational gap, rather than generating further in vitro efficacy data alone, is the field's central remaining task.
