Palliative care alongside cancer treatment improves quality of life, reduces depression, and in the landmark trial was associated
with living longer. The obstacle to using it earlier is not the evidence but the word, which patients and clinicians alike hear as an
announcement that treatment has stopped working.
Objectives: To review the published evidence on early integrated palliative care delivered alongside active cancer treatment:
what it changes, when it should begin, what families report afterwards, and how far the evidence extends beyond advanced
disease.
Material and Methods: Narrative review of randomised and cluster-randomised trials, meta-analyses, society guidance and
bereaved-caregiver studies indexed in PubMed and the major oncology and palliative medicine journals. No new patient data
were generated; every estimate quoted is one published by the original investigators.
Results: In the landmark randomised trial of 151 patients with newly diagnosed metastatic non-small-cell lung cancer, early
palliative care integrated with standard oncology improved quality of life at 12 weeks (mean FACT-L 98.0 against 91.5, p =
0.03), reduced depressive symptoms (16 against 38 per cent, p = 0.01), reduced aggressive care at the end of life (33 against 54
per cent, p = 0.05), and was associated with longer median survival (11.6 against 8.9 months, p = 0.02). Later
cluster-randomised and multicentre trials reproduced the quality-of-life findings without consistently reproducing the survival
difference.
Conclusion: The evidence supports early referral in advanced cancer and is strong enough to have changed guidelines. It does
not extend to patients receiving treatment with curative intent, where no randomised trial has been conducted, and the survival
finding in particular has not replicated reliably and should not be used as the reason for referral.
