Abstract

Two patients who meet the same classification criteria for early rheumatoid arthritis on the same day can go on to have almost nothing in common in their disease course once biologic therapy is started, yet the trial literature that established biologic efficacy in early disease was built around single-timepoint endpoints rather than the shape of each patient's disease activity curve over time. This article synthesizes the literature on disease activity trajectories in early rheumatoid arthritis treated with biologic agents, drawing together the measurement infrastructure that makes trajectories comparable across patients (the DAS28, EULAR response criteria, and ACR/EULAR remission and classification criteria), the treat-to-target and window-of-opportunity evidence explaining why early trajectory shape carries independent prognostic weight, the landmark early-RA biologic trials read through a trajectory lens, and the group-based trajectory modeling methodology now applied to real-world cohorts to characterize distinct disease-course subgroups. We give particular attention to predictors that distinguish rapid-remission from persistently active trajectories and to difficult-to-treat rheumatoid arthritis as the clinical phenotype at the persistent-activity end of the spectrum. Read together, the evidence supports early, target-driven, biologic-inclusive therapy as a strategy that shifts trajectories toward faster, deeper, more sustained improvement, while leaving substantial heterogeneity within every treatment arm studied to date, and points toward applying trajectory-modeling methods directly to existing trial data as a clear next step.
