Abstract

Osteoporotic hip fracture repair restores mechanical continuity to the femur, but bone mineral density (BMD) at the operated and contralateral hip often continues to decline for months to years afterward unless actively addressed. This article synthesizes the literature on BMD recovery patterns following hip fracture repair, covering the epidemiological scale of the problem, the biology of post-fracture bone loss and its coupling to muscle loss in osteosarcopenia, how BMD trajectory and fracture risk are measured with DXA and FRAX in this population, and what pharmacological, nutritional, and exercise-based interventions have been shown to modify that trajectory. We review evidence that antiresorptive therapy initiated promptly after hip fracture improves fracture and mortality outcomes with a broadly consistent BMD response across subgroups, that correcting the vitamin D and calcium deficits common in this population reduces subsequent hip fracture risk, and that progressive resistance training produces a measurable, site-specific BMD gain at the hip. We also review the organizational evidence for orthogeriatric co-management and fracture liaison services, the care models most consistently associated with translating this evidence into practice. Read together, the literature supports a reasonably well-characterized biological and clinical picture of modifiable post-fracture BMD decline, while leaving direct head-to-head pharmacological comparisons in post-hip-fracture cohorts, and the implementation of secondary-prevention pathways in lower-resource, higher-growth settings, as open priorities.
