Biosimilar substitution has moved from a theoretical regulatory category to a routine decision point in ambulatory pharmacy care, yet the pathway from biosimilarity to interchangeability to an actual pharmacy-counter substitution remains uneven and, in the United States, considerably more constrained than the accumulated clinical evidence would justify. This article synthesizes the real-world evidence and practice literature on biosimilar substitution in ambulatory pharmacy settings, tracing the regulatory logic that separates a biosimilar from an interchangeable biosimilar, the nocebo effect and communication factors that determine whether a clinically sound switch succeeds at the patient level, the accumulated randomized and real-world switching evidence across inflammatory disease, oncology, and endocrinology, the practice patterns and preparedness gaps documented among community and ambulatory pharmacists, and the policy and reimbursement levers that explain why biosimilar uptake varies so widely across countries with essentially the same underlying clinical evidence. Read together, the literature supports substitution as clinically sound and safety-neutral across the therapeutic areas studied at scale, while identifying pharmacist counseling structure, state-level substitution law clarity, and demand-side policy incentives, rather than any residual clinical uncertainty, as the binding constraints on realizing the full substitution opportunity in ambulatory practice.
