Abstract

Circulating tumor DNA (ctDNA) -- the tumor-derived fraction of cell-free DNA shed into blood plasma -- has emerged as a candidate biomarker for detecting molecular residual disease after curative-intent treatment for early-stage breast cancer, at a point when standard pathological and imaging-based staging cannot distinguish patients who are truly disease-free from those harboring undetectable micrometastatic disease. This article synthesizes the literature establishing ctDNA's prognostic value in this setting, from the foundational demonstrations that tumor-derived mutations are detectable and quantifiable in plasma, through personalized tumor-informed assays that track patient-specific mutation panels across serial postoperative and neoadjuvant blood draws, to a recent meta-analysis of 42 pooled studies reporting that ctDNA positivity carries a multivariate hazard ratio for disease-free survival rising from roughly 2.5 at pre-treatment baseline to more than 7 during postoperative surveillance, with detection preceding clinical relapse by a mean lead time exceeding ten months. We give particular weight to the assay-design and analytical-validity literature, since reported sensitivity varies by nearly an order of magnitude across studies depending on whether a personalized multiplex panel or a tumor-agnostic platform is used, and to the smaller but essential body of interventional trial evidence, exemplified by the c-TRAK TN trial, that tests whether acting on a positive ctDNA result actually improves outcomes rather than merely moving the diagnosis of an already-determined recurrence earlier in time. Read together, the evidence supports ctDNA as a genuine, reproducible, independent prognostic marker for recurrence in early breast cancer, while leaving demonstrated clinical utility, cross-platform assay standardization, and a fuller biological account of inter-patient shedding variability as the field's central open problems.
